1 पाचन का परिचय by RAS A2Z

मनुष्य द्वारा ग्रहण किया गया भोजन मुख्यतः कार्बोहाइड्रेट (Carbohydrates), प्रोटीन (Proteins), वसा (Fats), विटामिन (Vitamins), खनिज लवण (Minerals) तथा जल (Water) से मिलकर बना होता है।

कार्बोहाइड्रेट, प्रोटीन एवं वसा जैसे जटिल अणु सीधे रक्त में अवशोषित नहीं हो सकते। इन्हें एंजाइमों (Enzymes) तथा अन्य पाचक पदार्थों की सहायता से छोटे, घुलनशील और अवशोषण योग्य अणुओं में परिवर्तित किया जाता है।

इस संपूर्ण प्रक्रिया को पाचन (Digestion) कहते हैं।

पाचन का मूल क्रम

भोजन ग्रहण → यांत्रिक पाचन → रासायनिक पाचन → अवशोषण → परिवहन → कोशिकीय उपयोग → अपशिष्ट निष्कासन

2 पाचन के प्रमुख चरण by RAS A2Z

मानव पाचन प्रक्रिया को निम्न चरणों में समझा जा सकता है—

1. अंतर्ग्रहण (Ingestion)

भोजन को मुख द्वारा शरीर में लेना।

2. प्रणोदन (Propulsion)

भोजन को पाचन नली में आगे बढ़ाना।

इसमें निगलना (Swallowing) तथा क्रमाकुंचन (Peristalsis) महत्वपूर्ण हैं।

3. यांत्रिक पाचन (Mechanical Digestion)

भोजन को छोटे कणों में विभाजित करना, जैसे चर्वण (Chewing) एवं आमाशय का मंथन।

4. रासायनिक पाचन (Chemical Digestion)

एंजाइमों द्वारा जटिल भोजन को सरल पदार्थों में तोड़ना।

5. अवशोषण (Absorption)

पचे हुए पोषक तत्वों का रक्त या लसीका में प्रवेश।

6. आत्मसात/उपयोग (Assimilation)

अवशोषित पोषक तत्वों का कोशिकाओं द्वारा उपयोग।

7. मलत्याग (Defecation)

अपाच्य अवशेषों का शरीर से बाहर निकलना।

3 मानव पाचन तंत्र by RAS A2Z

मानव पाचन तंत्र (Digestive System) दो मुख्य भागों से बना है—

A. पाचन नली (Alimentary Canal)

क्रम—

मुख → ग्रसनी → ग्रासनली → आमाशय → छोटी आंत → बड़ी आंत → मलाशय → गुदा

B. सहायक पाचक अंग (Accessory Digestive Organs)

4 मुखगुहा — पाचन की शुरुआत by RAS A2Z

पाचन की शुरुआत मुखगुहा (Oral Cavity) से होती है।

यहाँ मुख्यतः तीन कार्य होते हैं—

  1. भोजन का ग्रहण
  2. दाँतों द्वारा चर्वण
  3. लार के साथ भोजन का मिश्रण

भोजन के छोटे कण बनने से पाचक एंजाइमों के लिए भोजन का सतह क्षेत्र बढ़ जाता है।

5 दाँत एवं उनका कार्य by RAS A2Z

वयस्क मनुष्य में सामान्यतः 32 स्थायी दाँत (Permanent Teeth) होते हैं।

6 लार ग्रंथियाँ by RAS A2Z

मनुष्य में तीन प्रमुख जोड़ी लार ग्रंथियाँ (Salivary Glands) होती हैं—

  1. कर्णपूर्व ग्रंथि (Parotid Gland)
  2. अधोजभ ग्रंथि (Submandibular Gland)
  3. अधोजिह्वीय ग्रंथि (Sublingual Gland)

इनसे लार (Saliva) स्रावित होती है।

लार के प्रमुख कार्य

7 लार एमाइलेज by RAS A2Z

लार में लार एमाइलेज (Salivary Amylase) या टायलिन (Ptyalin) पाया जाता है।

यह स्टार्च (Starch) जैसे जटिल कार्बोहाइड्रेट के पाचन की शुरुआत करता है।

स्टार्च → माल्टोज आदि छोटे कार्बोहाइड्रेट

महत्वपूर्ण बात यह है कि मुख में भोजन का पाचन सीमित अवधि तक ही होता है क्योंकि भोजन शीघ्र ही ग्रासनली और आमाशय में पहुँच जाता है।

8 ग्रसनी एवं ग्रासनली by RAS A2Z

मुख से भोजन ग्रसनी (Pharynx) में पहुँचता है।

इसके बाद भोजन ग्रासनली (Oesophagus) में प्रवेश करता है।

ग्रासनली का मुख्य कार्य भोजन को आमाशय तक पहुँचाना है। यहाँ महत्वपूर्ण पाचन नहीं होता।

भोजन को आगे बढ़ाने के लिए पाचन नली की पेशियाँ क्रमाकुंचन (Peristalsis) करती हैं।

9 क्रमाकुंचन by RAS A2Z

पाचन नली की दीवार की पेशियों का क्रमिक एवं लयबद्ध संकुचन-शिथिलन, जिससे भोजन आगे की ओर बढ़ता है, क्रमाकुंचन (Peristalsis) कहलाता है।

यह प्रक्रिया भोजन को गुरुत्वाकर्षण के अतिरिक्त सक्रिय रूप से आगे बढ़ाने में सक्षम बनाती है।

10 निगलने में एपिग्लॉटिस by RAS A2Z

एपिग्लॉटिस (Epiglottis) उपास्थि से बनी पत्ती जैसी संरचना है।

निगलने के समय यह श्वासनली (Trachea) के प्रवेश मार्ग को ढकने में सहायता करता है, जिससे भोजन सामान्यतः श्वसन मार्ग में जाने के बजाय ग्रासनली में चला जाता है।

भोजन का श्वसन मार्ग में जाना अभिलेपण/चोकिंग (Aspiration/Choking) का कारण बन सकता है।

11 आमाशय by RAS A2Z

आमाशय (Stomach) एक पेशीय, थैलीनुमा अंग है।

इसके प्रमुख कार्य—

आमाशय से बनने वाला अर्ध-द्रव भोजन काइम (Chyme) कहलाता है।

आमाशय की ग्रंथियों से आमाशयी रस (Gastric Juice) स्रावित होता है।

इसके प्रमुख घटक हैं—

आमाशय में उपस्थित हाइड्रोक्लोरिक अम्ल—

  1. अम्लीय वातावरण बनाता है।
  2. पेप्सिनोजेन को सक्रिय करने में सहायता करता है।
  3. कई सूक्ष्मजीवों के विरुद्ध सुरक्षा प्रदान करता है।
  4. प्रोटीन के विकृतिकरण (Denaturation) में सहायता करता है।

पेप्सिन का सक्रियण

पेप्सिनोजेन → हाइड्रोक्लोरिक अम्ल → पेप्सिन (Pepsin)

पेप्सिन प्रोटीन के पाचन में सक्रिय भूमिका निभाता है।

14 श्लेष्मा की भूमिका by RAS A2Z

आमाशय में अम्ल तथा प्रोटीन-पाचक एंजाइम उपस्थित होने के बावजूद आमाशय स्वयं सामान्यतः पचता नहीं है।

इसका एक महत्वपूर्ण कारण है—

श्लेष्मा-आधारित सुरक्षात्मक परत (Mucosal Barrier)।

यह आमाशय की आंतरिक सतह को अम्ल एवं पाचक पदार्थों से सुरक्षा प्रदान करती है।

श्लेष्मा सुरक्षा में बाधा होने पर पेप्टिक अल्सर (Peptic Ulcer) जैसी स्थितियाँ उत्पन्न हो सकती हैं।

15 आंतरिक कारक by RAS A2Z

आमाशय की कुछ कोशिकाएँ आंतरिक कारक (Intrinsic Factor) स्रावित करती हैं।

यह विटामिन B₁₂ (Vitamin B₁₂) के अवशोषण के लिए आवश्यक है।

विटामिन B₁₂ का प्रमुख अवशोषण अंतिम इलियम (Terminal Ileum) में होता है।

पेप्सिन प्रोटीन के पाचन में प्रमुख एंजाइम है।

यह मुख्यतः आमाशय की कुछ कोशिकाओं से स्रावित होता है।

छोटी आंत (Small Intestine) पाचन एवं अवशोषण का सबसे महत्वपूर्ण भाग है।

छोटी आंत में होता है।

आमाशय से काइम सबसे पहले ग्रहणी (Duodenum) में आता है।

यहाँ—

आमाशय से आने वाले अम्लीय काइम को क्षारीय बनाने में अग्न्याशयी बाइकार्बोनेट (Bicarbonate) अत्यंत महत्वपूर्ण है।

20 यकृत और पित्त by RAS A2Z

यकृत (Liver) शरीर की सबसे बड़ी ग्रंथियों में से एक है।

पाचन में इसका प्रमुख योगदान पित्त (Bile) का निर्माण है।

पित्त में सामान्यतः—

होते हैं।

पित्त में पाचक एंजाइम नहीं होते।

21 पित्त का पायसीकरण by RAS A2Z

पित्त लवण वसा की बड़ी बूंदों को छोटी बूंदों में विभाजित करते हैं।

इसे पायसीकरण (Emulsification) कहते हैं।

इससे—

वसा की सतह का क्षेत्रफल बढ़ता है → लाइपेज की क्रिया अधिक प्रभावी होती है।

पित्त वसा को रासायनिक रूप से नहीं तोड़ता।

यह मुख्यतः वसा का भौतिक पायसीकरण करता है।

22 पित्ताशय by RAS A2Z

पित्ताशय (Gallbladder) यकृत द्वारा निर्मित पित्त का—

करता है।

यकृत बनाता है → पित्ताशय संग्रह करता है → छोटी आंत में पित्त पहुँचता है।

23 अग्न्याशय by RAS A2Z

अग्न्याशय (Pancreas) एक मिश्रित ग्रंथि (Mixed Gland) है।

इसका—

बहिःस्रावी भाग (Exocrine Part)

पाचक एंजाइम एवं बाइकार्बोनेट युक्त अग्न्याशयी रस बनाता है।

अंतःस्रावी भाग (Endocrine Part)

इंसुलिन (Insulin), ग्लूकागॉन (Glucagon) जैसे हार्मोन बनाता है।

इस अध्याय में पाचन की दृष्टि से बहिःस्रावी कार्य विशेष महत्वपूर्ण है।

24 अग्न्याशयी रस by RAS A2Z

अग्न्याशयी रस (Pancreatic Juice) में प्रमुख रूप से—

25 एंजाइमों का सक्रियण — महत्वपूर्ण अवधारणा by RAS A2Z

अग्न्याशय कुछ प्रोटीन-पाचक एंजाइमों को निष्क्रिय अग्रदूतों के रूप में स्रावित करता है।

ट्रिप्सिनोजेन → एंटेरोपेप्टिडेज (Enteropeptidase) → ट्रिप्सिन (Trypsin)

इसके बाद ट्रिप्सिन अन्य प्रोटीज अग्रदूतों को सक्रिय करने में सहायता करता है।

अम्लीय काइम के ग्रहणी में पहुँचने पर सीक्रेटिन का स्राव बढ़ता है।

कोलीसिस्टोकाइनिन (Cholecystokinin, CCK) विशेष रूप से वसा एवं प्रोटीन युक्त भोजन की उपस्थिति में महत्वपूर्ण भूमिका निभाता है।

चरण 2 — एंजाइमिक पाचन

लाइपेज (Lipase) → वसा → वसीय अम्ल + मोनोग्लिसराइड

वसा के पाचन से बने उत्पादों का महत्वपूर्ण भाग आंत्र कोशिकाओं में काइलोमाइक्रॉन के रूप में पैक होने के बाद लसीका वाहिकाओं (Lymphatic Vessels) में प्रवेश करता है।

इसके बाद लसीका तंत्र उन्हें रक्त परिसंचरण में पहुँचाता है।

वर्मीफॉर्म अपेंडिक्स (Vermiform Appendix) सीकम से जुड़ी छोटी संरचना है।

इसे केवल "बेकार अंग" मानना वैज्ञानिक रूप से उचित नहीं है। इसमें प्रतिरक्षा-संबंधी ऊतक पाए जाते हैं और यह आंत्र माइक्रोबायोटा से भी संबंधित माना जाता है।

45 पाचन तंत्र का कार्यात्मक नियंत्रण by RAS A2Z

पाचन केवल एंजाइमों की शृंखला नहीं है। इसमें तीन स्तरों का समन्वय होता है—

1. तंत्रिका नियंत्रण (Neural Control)

आंत्र तंत्रिका तंत्र (Enteric Nervous System) पाचन नली की गति एवं स्राव को नियंत्रित करता है।

2. हार्मोनल नियंत्रण (Hormonal Control)
